Headquartered in Louisville, Kentucky, Humana, Inc. has been around for years and is one of the largest
publicly traded health insurance companies. Humana Insurance Company is rated "A-" (Excellent) by AM Best.
Humana utilizes the ChoiceCare network which includes more than 320,000 physicians and ancillary care
providers in 50 states. Like other plans, Humana's PPO network can provide substantial discounts for the
member while the deductible is being met. Please note that the Humana PPO provider network is inadequate
in the following counties in Colorado: Custer, Dolores, Hinsdale, Jackson, Mineral, San Juan, San Miguel.
HumanaOne Health Insurance products are competitively priced with a great line up of Traditional PPO
and HSA options. Dental plans can be purchased along with Health Insurance but must be done so at the
time health insurance is purchased. You can choose any Dentist but maximum benefit is paid to Dentists
in their PPO network which numbers about 70,000 nationwide. The HumanaOne dental plan includes a limited
benefit for teeth whitening services.
As with all health insurance carriers in the individual insurance market, HumanaOne has the right to
accept, decline or waiver (not cover) any condition. Let us help guide you through the process especially

In addition to their Health Insurance plans, HumanaOne members can also select an optional maternity
benefit which is designed to help cover the cost of routine pregnancy and delivery. The pregnancy rider:
- Includes Prenatal
- Has an additional $500 deductible, and
- Pays up to 60% of the costs of routine pregnancy and childbirth following the deductible.
- The rider premium is approximately $107.00 per month, depending upon your age.
- Maternity coverage is not available with an HSA plan
- There is a full 12 month waiting period before the plan will pay on this maternity benefit.
